Bad tape

Post Bad tape 
I have a tape that will not mount in my library. NBU requests the tape and it loads into the drive, but for whatever reason it sits in the library at the "Rewound" state (normally it would be "Loaded"). NBU never thinks the tape is loaded so it's still waiting for it to mount.
Does anyone have any tricks or ideas for me to get this tape to load or otherwise recover the data to another tape?

Post Bad tape 
You didn't specify the tape but if it's a single-reel type, like LTO or DLT,
then the tape leader might be broken off. This can be reattached by a
tech-type.
